TFT stands for Thin Film Transistor. These are a sub-type of LCD screens, rather than a separate entity; TFT type screens provide the highest resolution of all LCD screens. TFT monitors are sometimes called Active Matrix monitors; the other main kind of LCD technology is Passive matrix, which is lower quality, cheaper, and to my knowledge in lower use nowadays.

A TFT screen is an LCD screen. Some LCD screens use TFT technology, and some don't. TFT technology provides the best resolution of all the flat screen techniques, but is also the most expensive. TFT LCD screens are also called active matrix LCD screens.

There is no difference. You will often see monitors listed as "TFT LCD Monitor".

TFT is the current technology used for LCD screens, there used to be others such as DSTN which are obsolete. They are for all intents and purposes the same if you are using or purchasing one today.
